PLEASE NOTE:  In order to avail of available grants and services all people with Cystic Fibrosis (PWCF) must fill out the membership form below, get it signed/stamped by their CF consultant and send it back to the CF Ireland, 24 Lower Rathmines Road, Rathmines, Dublin 6.
